The following checklist is for those students who matriculated between Fall 1986 and Spring 2000

Accounting, Health Science, Nursing and other Bachelor of Science students should see a counselor in the Office of Student Services and/or their Departmental advisor. Your requirements are different than those listed in this checklist.

1. Fill out the Distribution Checklist portion by clicking on the menu options.

2. Print out the form.

3. Fill in the Pluralism and Diversity portion by hand.

4. Bring to Counselor in Student Services - Room 1119 Hunter East
